Using an online form - Accurx

We are pleased to offer an online platform for our patients to contact the practice with non-urgent medical or administrative requests. All queries will be reviewed by one of our team within 5 working days. We will only respond to requests within our core working hours.
The service will be unavailable when the surgery is closed (This includes weekends and Bank Holidays)

  • Do not use this service if you need a book on the day appointment or home visit – please contact the surgery by telephone
  • Do not use this service to request your prescription medication – continue to use our online prescription request service
  • Do not use this service to chase hospital appointments or test results carried out by the hospital – please contact your hospital specialist directly for these requests
  • Do not submit any information that you are not willing to appear in your primary care records as all requests will be saved into your records along with our response.

When submitting a request please remember to answer the questions as thoroughly as possible. This will enable our clinicians and clerical team to respond to your request appropriately.

Do not submit any third-party information (information about a family member, relative or friend)

Using your NHS account/NHS App

There are other tasks you can do online, like:

  • order repeat prescriptions
  • view your test results
  • see parts of your health record
  • check or cancel appointments
  • see your vaccination record

For these, you can use your NHS account/NHS App

Setting up an NHS account NHS App

To set up your NHS account for the first time, you will need to set up an NHS login. Read about how to set up NHS login (nhs.uk).

Once you have set up your NHS login, you can log in to your NHS account or download the NHS App onto your phone.
